Inappropriate CAP diagnosis common in hospitalized adults
A prospective cohort study of 48 hospitals in Michigan found that 12% of patients hospitalized with community-acquired pneumonia (CAP) appeared to have been diagnosed incorrectly and that of these, 87.6% received a full course of antibiotics.

Antipsychotic medications not effective in ICU patients with delirium

Palliative care consults by default did not affect inpatient length of stay
Implementation of a default, opt-out palliative care consultation intervention for older hospital patients with advanced chronic illnesses did increase consult rates from 16.6% to 43.9% and significantly increased odds of discharge to hospice.

Study of Medicare claims finds good outcomes with hospital at home
Only 6.2% of Medicare fee-for-service patients who were hospitalized at home had to be escalated to another setting and 0.5% died during the admission, according to an analysis of 2022 to 2023 claims data. Their 30-day readmission rate was 15.6%.

ED visits after discharge to SNF cost Medicare $24 million in 2019
Sixteen percent of Medicare patients who were discharged to a skilled nursing facility (SNF) came back to the ED within two weeks, and a quarter of those visits resulted in discharge back to the SNF the same day.
