High-sensitivity troponin-based pathway safe, effective for chest pain patients without CAD

The American College of Cardiology expert consensus decision pathway for chest pain did not meet the criteria for safety among patients with known coronary artery disease (CAD), according to a study conducted in U.S. EDs.

3-hour troponin level may help rule out NSTEMI in ED patients triaged to observation

A previously proposed cutoff for 3-hour change in high-sensitivity cardiac troponin T of 7 ng/L missed 66.7% of ED patients with non-ST-segment elevation myocardial infarction (NSTEMI), but a cutoff of 15 ng/L combined with a 0/3-hour absolute change of less than 4 ng/L missed less than 1%.

High-sensitivity cardiac troponin may be useful for early ruleout of MI

A Scottish study found that length of stay decreased and discharge rates increased when a high-sensitivity cardiac troponin I concentration below 5 ng/L at ED presentation was used to rule out myocardial infarction (MI), although the safety of this strategy could not be proven.

High-sensitivity troponin testing improved processes, outcomes, hospital studies find

One study found that implementation of high-sensitivity testing reduced five-year mortality among patients with nonischemic myocardial injury, while another showed that it increased serial troponin testing but not hospital admissions.

Elevated troponin I levels during sepsis may indicate risk for postsepsis CV complications

A U.S. retrospective study looked at rates of atherosclerotic disease, atrial fibrillation, and heart failure in the year after sepsis in patients with elevated troponin I levels and no previous diagnosis of cardiovascular (CV) disease.

Catheters recalled; warning on troponin tests

The FDA announced a recall of certain peripheral catheters due to leaking and warned not to use a cardiac testing panel providing inaccurate troponin measurements, among other recent actions.

High-sensitivity troponin reduced the cascade of cardio interventions after chest pain

An analysis of ED patients found an increase in repeat troponin testing and EKGs, but reductions in CT scans, stress tests, cardiology evaluations, and hospitalizations associated with implementation of high-sensitivity cardiac troponin testing.

Elevated troponin levels associated with increased mortality in all age groups

Troponin was directly associated with mortality in patients without acute coronary syndrome and those with acute coronary syndrome who were managed medically. In patients treated invasively, there was a paradoxical decrease in mortality risk at the highest troponin levels.
