Do-not-attempt-resuscitation order rates compared for heart failure, other hospital conditions

A retrospective study in California found low overall rates of do-not-attempt-resuscitation orders in patients hospitalized for heart failure, pneumonia, acute myocardial infarction, or chronic obstructive pulmonary disease, with some variation by race.

High-intensity medical therapy at HF discharge reduced readmissions, death

Researchers stopped the STRONG-HF randomized trial early due to greater than expected differences in outcomes between patients who received rapid up-titration of guideline-directed medical therapy under strict follow-up and those who received usual care.

Algorithm for HF risk stratification in the ED associated with better 30-day outcomes

A trial in Ontario used a point-of-care algorithm to determine whether patients seen in the ED for acute heart failure (HF) should be discharged directly from the ED or after up to three days of observation, followed by transitional care in a heart failure clinic, or be admitted for inpatient care.

Acetazolamide improved decongestion in acute decompensated heart failure

A randomized trial found that the drug, already approved for other indications, achieved successful decongestion within three days in 42.2% of acute decompensated heart failure patients, compared to 30.5% of those on placebo.

Providing clinicians with prognostic data did not affect heart failure care

Readmissions and mortality were not significantly different between hospitalized heart failure patients who received care as usual and those whose records displayed an alert containing predicted risk of one-year mortality.

Meth use associated with higher risk of heart failure, myocardial infarction, stroke

An analysis of hospitalized patients found that those who used methamphetamine had a 32% increase in cardiovascular disease risk compared to nonusers who also had been hospitalized. Risk increases associated with misuse of alcohol and cocaine were 28% and 47%, respectively.

Latest COVID-19 research looks at lung ultrasound, CVST and VTE risk, visitation policies

Recent studies offered a new method for using lung ultrasound to assess COVID-19 patients, assessed risk of central venous sinus thrombosis (CVST), and looked at a lab test to help predict venous thromboembolism (VTE) and at the effects of restricting visitors.
