Natriuresis good indicator of decongestion in acute decompensated HF, trial of acetazolamide shows
Patients with decompensated heart failure (HF) who were randomized to acetazolamide had higher urine sodium concentrations (UNa) than those on placebo, and every 10-mmol/L increase in UNa was associated with a lower risk of all-cause death or HF readmission, a prespecified analysis found.

Procalcitonin algorithm reduced antibiotic use in COVID-19 inpatients
A Dutch intervention used procalcitonin levels in inpatients with COVID-19 to guide antibiotic use, with an algorithm discouraging prescriptions if levels were less than 0.25 ng/mL and recommending antibiotics for patients with levels above 0.5 ng/mL.

New consult service dramatically increased use of medication for alcohol use disorder
A third of inpatients with alcohol use disorder who were seen by an addiction consult service during their stay received a new prescription for related medication, compared to less than 1% of similar patients hospitalized before the service launched.

Hospital rankings change when Medicare Advantage patients included
When researchers included both Medicare fee-for-service and Medicare Advantage beneficiaries in calculations of CMS performance measures, between 21.6% and 30.2% of hospitals in the top-performing group for readmissions and mortality fell to a lower-performing group.

Transitioning to oral therapy for infective endocarditis supported by real-world study
A retrospective study at three L.A. hospitals found similar rates of clinical success at 90 days in patients who received entirely IV therapy or were transitioned to oral antibiotics for infective endocarditis, and adverse events were less common in the latter group.
