Off-label dosing of DOACs at discharge common in afib patients
More than 10% of patients hospitalized with afib receive a direct-acting oral anticoagulant (DOAC) at less or more than the recommended dose at discharge, according to data from the Get With The Guidelines-Atrial Fibrillation registry.

1 in 3 adults with new-onset a fib during hospitalization have recurrence within a year
The findings suggest that patients who experience a transient episode of atrial fibrillation during hospitalization need risk stratification and follow-up, according to the authors of the matched cohort study.

Use of decision aid in ED for patients with AF linked to decrease in admissions
Hospitals that implemented a decision aid to risk stratify patients with atrial fibrillation (AF) in the ED saw a 17% reduction in their odds of hospital admission, but also a small but significant increase in repeat presentations for AF within 30 days, a study found.

Inpatient diagnosis of atrial fibrillation associated with lower use of anticoagulation
Ninety days after being diagnosed with atrial fibrillation, 51.6% of patients diagnosed in the ED were on oral anticoagulants, compared to 50.9% of those diagnosed in the hospital and 67.9% of those diagnosed as outpatients, a Canadian study found.

Early cardioversion for newly diagnosed nonvalvular afib may lower mortality
Only 14.9% of patients received early cardioversion in the international registry study, which also found no major difference in outcomes by whether patients received direct-current cardioversion or pharmacological cardioversion.

Troponin level may help predict mortality in patients admitted with afib
Troponin levels were associated with risk for death, and coronary angiography was associated with both higher troponin levels and lower mortality.

Atrial fibrillation common, increasing among patients hospitalized for COPD
In-hospital mortality was higher among patients who had atrial fibrillation in addition to chronic obstructive pulmonary disease (COPD), but it decreased significantly over time, from 6.1% in 2003 to 2.9% in 2014.

Anticoagulation didn't benefit patients with afib secondary to ACS, pulmonary disease, or sepsis
Only a third of the patients were discharged on anticoagulation, and over a three-year follow-up period, receiving anticoagulation was not associated with a significant reduction in ischemic stroke.
