Preop hyperglycemia but not HbA1c associated with noncardiac surgery outcomes
Patients with a high fasting or random glucose level before noncardiac surgery were more likely to have myocardial injury or die at 30 days, but no increased risk was seen among those with a high HbA1c level, a South Korean study found.

Study finds higher thresholds of troponin I associated with death after cardiac surgery
The troponin level one day after surgery associated with higher risk for death at 30 days was 218 times the upper reference limit (URL) after isolated coronary artery bypass grafting or aortic valve replacement or repair and 499 times the URL after other cardiac surgery.

DOACs better than heparin for periop thromboprophylaxis, review finds
The systematic review and network meta-analysis found lower rates of symptomatic venous thromboembolism in surgical patients given direct oral anticoagulants (DOACs) versus the standard dose of low-molecular-weight heparin, without a significant increase in bleeding.

Low-value preop screening frequently led to additional low-value care, study finds
Chest X-rays and EKGs of low-risk patients before low- or intermediate-risk surgery were followed by high rates of repeat tests, follow-up imaging, and follow-up visits, according to a retrospective analysis of Veterans Health Administration data.

Functional limitations associated with higher mortality risk after emergency surgery
In an analysis of Medicare patients admitted for emergency general surgery in 1992 through 2013, the presence of chronic conditions or geriatric syndromes at baseline did not predict mortality, but patients who also had functional limitations had higher risk of death after surgery.
