Communications interventions at discharge may improve readmissions, adherence
A meta-analysis showed that readmission rates among patients who received communication interventions at discharge were 9.1% compared to 13.5% among those who did not. Patient satisfaction was also significantly higher: 60.9% were satisfied versus 49.5% of controls.

Avoiding SNFs and antibiotics, expecting long-term effects and boosters in COVID-19
One study showed how a new committee reduced discharges to skilled nursing facilities (SNFs) while another indicated that empiric antibiotics aren't necessary for ventilated COVID-19 patients. Officials announced plans for vaccine boosters, and convalescent plasma failed an outpatient trial.

Care coordination program linked to reduced readmission rates in large health system
A study of 21 hospitals in an integrated health system in California found that a postdischarge intervention targeted at patients at 25% or higher predicted risk of 30-day readmission or mortality was associated with decreased rates of readmission, but not mortality, in the 30 days after discharge.

Discharge of heart failure patients to settings not recommended by therapists linked to increased readmission risk
Patients hospitalized with congestive heart failure who were discharged to a less intensive postacute setting than that recommended by physical and occupational therapists were more likely to be readmitted within 30 days than those discharged according to recommendations.

Pulmonary rehabilitation associated with fewer readmissions for COPD patients
Only 1.5% of studied patients began pulmonary rehabilitation within 90 days of discharge from a hospitalization for chronic obstructive pulmonary disease (COPD), but their rate of readmission within a year was 56.4%, compared to 64.6% in patients who did not begin rehab.

Meth-associated HF hospitalizations, costs rose sharply in recent years
From 2008 to 2018 in California, hospitalizations for methamphetamine-associated cardiomyopathy/heart failure (HF) increased by 585%, leading study authors to call for a “public health and clinical care response campaign to combat this multifaceted, fast-moving epidemic.”

Care pathway with nurse screening for delirium may have reduced length of stay, 30-day readmissions
An academic tertiary care hospital in Northern California found that adjusted mean hospital length of stay decreased by 2% and the adjusted odds of 30-day readmission decreased by 14% after implementation of a delirium screening and care pathway for inpatients ages 50 years and older.

Beta-blockers after discharge associated with improved outcomes in older patients with heart failure
Ninety-day mortality and readmission rates were significantly lower in Medicare patients who received a beta-blocker after hospital discharge for heart failure with reduced ejection fraction than in those who did not, even if they were 75 years of age and older.
