While rates of clinical failure for C-reactive protein (CRP)-guided antibiotic treatment duration and fixed seven-day treatment were noninferior to fixed 14-day treatment in a randomized trial, interpretation of the results was limited by a large noninferiority margin and low event rate.

After detailed risk adjustment, both inadequate and unnecessarily broad empiric antibiotic therapy were associated with higher mortality in patients admitted with culture-positive community-onset sepsis.
